History of Illness:

A woman aged 67 years who had suffered Parkinson's disease for 15 years, developed fever, cough with yellow sputum ("phlegm") for 3 days, and breathlessness for 2 days. These symptoms were associated with a sharp, stabbing pain over her left lower chest that was worse when she took a deep breath. She was admitted to hospital with signs of septicaemia - high fever, increased heart rate (130/min) and decreased blood pressure (80/50). There was decreased air entry and a pleural rub over her left lower chest. She died of septic shock the same day .
