The microscopic picture is typical of tuberculosis, with numerous granulomas (focal collections of chronic inflammatory cells) exhibiting central caseation (unstructured necrosis which appears cheesy white to the naked eye), surrounded by lymphocytes, epithelioid cells and Langhans-type giant cells. The periphery of these lesions shows fibrosis and proliferation of capillaries.
